Three 12-hour shifts with rotating weekends - specific day schedule to be determined. Shift from 8pm to 8:30am.

Under the directives of a physician and the supervision of a Registered Nurse (RN), the Continuous Care (CC) Licensed Practical Nurse (LPN) plays a vital role in monitoring patient and family needs while delivering supportive care and emotional assistance during critical times to enhance patient comfort and improve quality of life.

Key Responsibilities:

  • Following the guidance of the RN case manager, provides ongoing bedside clinical assessments, administers prescribed care, and offers both physical and emotional support to patients as outlined in the care plan, promptly reporting any significant changes in patient status or family requirements.
  • In partnership with the RN, processes and implements physician orders to ensure optimal patient comfort.

Qualifications:

  • Candidates must have graduated from an accredited nursing program and possess a valid Florida LPN license.
  • A minimum of one year of experience in an acute care environment, delivering direct patient care; prior experience in hospice care as an LPN is preferred.
