Job Overview

The Care Coordinator will provide health services and direct care to individuals with disabilities living in a residential program.

Key Responsibilities:

  • Provide direct specialized nursing and other day-to-day care to individuals.
  • Document healthcare services in accordance with state regulations, nursing standards, and Massachusetts LPN license requirements.
  • Liaise with community health care providers, monitor medical records, and administer medications.

Requirements:

  1. Graduation from an accredited licensed practical nurse program.
  2. Current Massachusetts Nursing license.
  3. Two years general nursing experience, preferably in a community health setting.

Salary Range: $55,000 - $65,000 per year
