Job Summary:
We are currently seeking a qualified Licensed Practical Nurse (LPN) to join our team in a residential program. As an LPN, you will play a vital role in providing direct care and health services to individuals with disabilities, ensuring their physical and emotional well-being.
Key Responsibilities:
- Deliver direct nursing care and day-to-day support to individuals
- Develop and maintain relationships with community healthcare providers and family members
- Collaborate with the interdisciplinary team to coordinate care and achieve optimal outcomes
- Ensure adherence to MAP policies and procedures for medication administration
