Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Kanza Mental Health. As a Case Manager, you will play a vital role in providing comprehensive support services to individuals experiencing serious mental illness.

Key Responsibilities
  • Provide case management services and access to community resources to support clients in their homes and communities.
  • Develop and implement comprehensive treatment plans in collaboration with clients, their families, and other members of the treatment team.
  • Provide psychoeducation and skill-building services to clients on an individual and group basis.
  • Coordinate and monitor all aspects of clients' care, including medical and psychiatric services, housing, employment, and other community-based resources.
  • Advocate for clients' needs and rights, including access to appropriate services and benefits.
  • Maintain accurate and up-to-date documentation of all client interactions and progress toward treatment goals.
Requirements
  • BS or BA degree in a Human Services field or 4 years of education/experience in the human services field.
  • Must be 21 years of age.
  • Successfully pass all background checks.
  • Valid Driver's License.
  • Strong organization skills.
  • General knowledge of computers.
  • Proficient in the English language.
Working Environment

The Case Manager will work in a typical office environment, school classroom, or home environment. The position requires the use of a company vehicle for business and travel to multiple worksites, including other offices, residences, and various places around the community.

The Case Manager may be exposed to low socio-economic homes and aggressive situations. The work schedule will generally conform to that of a full-time Monday-Friday 40-hour-per-week schedule.
