Job Overview

POSITION TITLE: Case Manager - Community Based Services

REPORTS TO: Community Based Services Supervisor

DIRECT REPORTS: None

FLSA STATUS: Non-exempt; regular full-time; employment-at-will position.

SCHEDULE: The work schedule will generally align with a full-time Monday-Friday 40-hour-per-week framework within the operational hours of the Center.

OBJECTIVE: The CBS (Community Based Services) Case Manager is tasked with delivering comprehensive in-person case management services to youth in various environments including home, school, or community settings. This role requires collaboration with a multidisciplinary team to enhance the mental health and overall well-being of youth clients, ensuring they access suitable services and resources to support their needs. Responsibilities include planning, coordination, monitoring, and advocacy to assist clients in achieving their treatment objectives and improving their quality of life.

KEY RESPONSIBILITIES:

  1. Deliver direct services in clients' homes, educational institutions, and community settings.
  2. Design and execute personalized service plans in partnership with clients and their families.
  3. Advocate for clients' requirements, aiding them in accessing community resources and services.
  4. Facilitate connections to crisis intervention services and assist in safety planning with the crisis team as necessary.
  5. Support clients in developing and enhancing essential life skills, social skills, and coping mechanisms.
  6. Collaborate closely with community partners, healthcare providers, and educational staff.
  7. Engage in treatment team meetings to review client progress and modify service plans as needed.
  8. Coordinate with both internal and external service providers to ensure holistic care.
  9. Maintain professional relationships with all partners and agencies involved in clients' care.
  10. Accurately and promptly complete all required client service documentation.
  11. Uphold client confidentiality and adhere to all ethical and legal standards.
  12. Document all services rendered, including client interactions, referrals, and follow-up activities.
  13. Participate in all mandatory training sessions and professional development opportunities.
  14. Stay updated on best practices and advancements in mental health and substance use disorder treatment.

REQUIRED QUALIFICATIONS:

  1. Bachelor's degree in human services preferred or significant relevant experience in the human services field. Alternatively, four years of education/experience in the human services sector.
  2. Must be at least 21 years of age; proficient in English; able to pass pre-employment background and drug screening; must complete case management and crisis training as mandated by the State of Kansas within the first 60 days of employment.
  3. Experience working with individuals facing Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), or Substance Use Disorders (SUD) is preferred.
  4. Strong organizational and time management capabilities.
  5. Excellent verbal and written communication skills.
  6. Ability to work independently as well as collaboratively within a team.
  7. Proficient in utilizing computers and electronic health record systems.
  8. Required to maintain professional boundaries and confidentiality.
  9. Individuals must be physically capable of operating a company vehicle safely, possess a valid driver's license, and maintain an acceptable driving record.

PHYSICAL REQUIREMENTS:

  1. Ability to remain stationary, such as standing or sitting, for extended periods.
  2. Frequent movement between various worksites to complete tasks.
  3. Adjusting or moving objects weighing up to 10 pounds in any direction.
  4. Repetitive motions that may involve the wrists, hands, or fingers.
  5. Effective communication with others to exchange information.
  6. Ability to withstand outdoor elements such as temperature, precipitation, and wind.
  7. This position necessitates the use of a company vehicle.

WORK ENVIRONMENT: The role is typically performed in an office environment, school classroom, or home setting. While on location, work is generally conducted indoors with standard room temperatures, lighting, and office equipment. This position will require travel to multiple worksites, including various community locations. Case Managers may encounter low socio-economic homes and potentially challenging situations.
