As part of our Community Medicine Program, the Care Transition Specialist plays a vital role in ensuring a seamless transition of patient care from hospital settings to home environments. Reporting to the Manager of Transportation and EMS, this position serves as a crucial link between patients, their primary care providers, and hospital staff, facilitating ongoing recovery and health education.

Key Responsibilities:

  • Facilitate the onboarding of new patients into the Community Medicine Program led by VNS
    • Engage with patients in the hospital to provide education and schedule their initial home visit post-discharge.
    • Conduct regular and ad-hoc home visits to support patients with health literacy and resources to prevent re-hospitalization.
    • Assist patients in arranging follow-up telehealth appointments with their primary care providers.
  • Support patients at home during emergencies or in conjunction with telehealth services provided by their primary care providers
    • Stabilize patients at home and coordinate with primary care for evaluations.
    • Perform assessments, gather vital signs, and relay essential information to the primary care team.
    • Assist in virtual evaluations during telehealth appointments.
Qualifications:
  • EMT-B Certification and CPR Certification required.
  • Valid Driver's License.
  • Strong understanding of anatomy and physiology related to patient care.
  • Excellent written and verbal communication skills for effective documentation and collaboration with medical professionals.
  • Ability to manage time and patient schedules independently.
Preferred Qualifications:
  • AEMT certification is preferred.
  • Minimum of 2 years of experience in an ambulance setting (active or volunteer).
Physical Requirements:
  • Must be able to drive, maintain various physical positions, and lift up to 50 pounds regularly.
Work Environment:
  • Position involves both hospital and patient home visits.
  • Daily travel between the hospital and patient residences is necessary.
Compensation: $30.20 per hour
