RN Clinical Care Coordinator Primary Care

We are seeking a dedicated RN Clinical Care Coordinator to join our Primary Care team. As a key member of our team, you will play a vital role in supporting our physicians and mid-level providers with clinical activities and direct patient care.
Responsibilities of the RN Clinical Care Coordinator include:
- Providing professional nursing care in accordance with New York State Nursing Practice Act
- Carrying out assigned nursing procedures while maintaining a calm and professional demeanor during emergent situations
- Providing patient education as requested by providers
- Preparing patient records for visits, including chart preparation and nurse note documentation
- Participating in scheduled team meetings to discuss clinical roles, processes, and patient concerns
- Working collaboratively with team members to achieve patient-centered care and quality by implementing best practice standards
The shifts for this RN position will range from 7:30am-5:30pm, Monday through Friday.
Requirements for the RN Clinical Care Coordinator position include:
- Graduation from an accredited RN program
- Current licensure as a registered nurse in New York State
Preferred qualifications include:
- Basic knowledge of cardiology diseases and treatment
- Knowledge of medical insurance carriers and prior authorization processes
- Ability to perform manual blood pressure checks
