Role Summary:

The Clinical Support Manager (RN) collaborates closely with the Senior Director of Operations to ensure that all nursing care and services are effectively coordinated and managed in accordance with the standards of Hospice of Holland.

Key Responsibilities:

  • Participates in the on-call nursing supervisory rotation during weekends, evenings, and off-hours, serving as a backup for other Clinical Managers.
  • Assists the Senior Director of Operations in overseeing all Clinical Care staff, ensuring quality clinical care is delivered by on-call nurses and hospice aides.
  • Works with the clinical services team to align with the strategic goals of Hospice of Holland, identifying areas for improvement and addressing customer service issues as necessary.
  • Ensures compliance with local, state, and federal regulations in nursing practices, participating in Medicare surveys as required.
  • Responsible for evaluating clinical care services provided by staff under supervision, promoting high-quality and safe patient care.
  • Conducts reviews of team competencies through home visits, care conferences, and documentation assessments, remaining available to assist clinicians during operational hours.
  • Reviews patient medical information to ensure effective and compliant clinical care, facilitating interdisciplinary team meetings.
  • Assists clinicians in determining immediate care needs during off-hours and manages call system activities.
  • Supports the Senior Director of Operations in the recruitment and onboarding of new staff members.
  • Contributes to the development and revision of nursing policies and procedures.
  • Ensures adherence to all relevant laws and regulations regarding clinical care.
  • Assists with the staffing and scheduling of clinical care personnel efficiently.
  • Ensures that clinical documentation in the electronic medical record meets established standards.
  • Oversees the management of medical supplies and equipment, ensuring compliance with organizational policies.
  • Strives to guarantee that every individual under the care of Hospice of Holland receives optimal support.

Qualifications:

  • Graduate of an accredited nursing program with a BSN degree; a master's degree is preferred.
  • Experience in hospice or home care settings with at least two years of clinical nursing experience; supervisory experience is preferred.
  • Current Registered Nurse licensure in the state of Michigan; must possess a valid driver's license and insured vehicle.
  • Demonstrated leadership abilities, capable of fostering a motivated and high-performing team.
  • Strong nursing skills and a deep understanding of hospice care principles.
  • Ability to analyze and interpret various professional documents and regulations.
  • Proficient in report writing and business correspondence.
  • Effective in presenting information in both individual and small group settings.
  • Skilled in problem-solving, data collection, and drawing valid conclusions.
  • Knowledgeable about professional standards and practices in nursing.
  • Adept at managing change and multitasking in a dynamic environment.
