Job Overview

In this role, you will operate under the guidance of the Clinical Health Team Supervisor, delivering essential primary health services in family planning, women's health, and sexual wellness. Your responsibilities will encompass the medical oversight of health issues, including the authority to prescribe and manage medications. You will engage in health evaluations, education, counseling, direct patient care, referrals, and case management to enhance health outcomes for individuals and families with limited access to healthcare services. Additionally, you will provide expert consultation to fellow clinic personnel.

This part-time position requires a commitment of 24 hours per week and offers benefits, based at the Public Health facility in Holland, Michigan. You may be required to travel to various County locations as necessary. Standard operational hours are Monday through Friday, with some flexibility on Tuesdays.

Key Responsibilities

The primary duties of this position include, but are not limited to:
  1. Delivering managed care by creating and executing personalized treatment plans and conducting follow-ups with other healthcare professionals and community organizations.
  2. Evaluating and documenting presenting health issues, risk factors, family medical history, and psychosocial backgrounds.
  3. Conducting thorough physical examinations and assessments.
  4. Ordering and performing necessary diagnostic tests and gathering additional information for health evaluations.
  5. Formulating differential diagnoses and identifying risks through comprehensive assessments.
  6. Prescribing medications and overseeing their administration.
  7. Providing case management services, developing and monitoring care plans, and ensuring proper documentation of follow-up services; advocating for clients and making appropriate referrals.
  8. Maintaining accurate patient records and ensuring confidentiality of client information.
  9. Collecting health histories through interviews and chart reviews, compiling data for quality assurance purposes.
  10. Ensuring compliance with established professional standards, ethical guidelines, and departmental policies.
  11. Developing clinical protocols in accordance with relevant standards and guidelines.
  12. Performing other related duties as assigned.

Qualifications

To be considered for this position, candidates must possess a Master of Science in Nursing (M.S.N.) from an accredited institution, Board certification as a Nurse Practitioner, and a minimum of eighteen months of relevant clinical experience.

Licenses and Certifications:

Valid State of Michigan licensure as a Registered Nurse and Specialty certification as a Nurse Practitioner are required.

Knowledge and Skills Required:
  1. Comprehensive understanding of advanced nursing practices.
  2. Familiarity with medical record documentation standards.
  3. In-depth knowledge of diagnostic testing and result interpretation.
  4. Awareness of federal, state, and local regulations governing client services, including relevant standards and guidelines.
  5. Understanding of healthcare resources available to client populations.
  6. Strong organizational skills with the ability to prioritize tasks effectively.
  7. Ability to interact positively with diverse individuals across various disciplines.
  8. Excellent communication skills, both written and verbal.
