Job Title: Stroke Program Coordinator

We are seeking a highly skilled and experienced Stroke Program Coordinator to join our team at Hialeah Hospital. As a key member of our clinical administration team, you will play a vital role in ensuring the highest quality care for our stroke patients.

Job Summary:

The Stroke Program Coordinator will be responsible for coordinating all aspects of patient care related to stroke, including reviewing current program standards, updating knowledge on changes that affect the stroke program, and maintaining ongoing daily stroke patient lists. You will also collaborate with the Neuroscience APRN in developing outcome-driven PI indicators and attend ED and IP code strokes.

Key Responsibilities:
  • Review current program standards related to certification and update knowledge on changes that affect the stroke program.
  • Concurrently chart review on all stroke cases to identify opportunities for improvement according to latest AHA/TJC standards.
  • Maintain ongoing daily stroke patient list for review while maintaining highest level of following all required stroke indicators with multidisciplinary team.
  • Collaborate with Neuroscience APRN in developing outcome-driven PI indicators.
  • Attend ED and IP code strokes.
  • Point of contact in healthcare system for stroke program.
  • Community Education/Outreach on Stroke following AHA guidelines.
  • Staff Education on Stroke (Nursing and Medical Staff) using AHA guidelines using different methods of instruction.
  • Attend General Orientation to educate all new staff with stroke and the Comprehensive Stroke Program.
  • Attend Nursing orientation to educate all new nurses on the Comprehensive Stroke Program nursing responsibilities.
  • Attend regional meetings on Stroke.
  • Data entry- GWTG.
  • Attend stroke program meetings.
  • Attend core team meetings.
  • Collaborate with interdisciplinary team when opportunities are identified in an effort to correct issues.
  • Provides feedback to the staff on new measures and initiatives related to the stroke program, provides staff education to assure compliance.
  • Active participant of the stroke collaborative, AHA get with the guidelines and other community-based stroke groups.
  • Active reporting member of the Comprehensive Stroke Committee meeting in conjunction with Stroke medical director.
  • Maintains survey readiness at all times.
  • Complies with regulatory readiness at all times by providing required documents and attending required training.
  • Assists with TJC surveys as required.
  • Assists with AHCA surveys as required.
Requirements:
  • Five or more years of clinical experience in ED, Critical Care, or Neuro ICU Unit.
  • Basic computer knowledge.
  • Knowledge of general nursing theories and practice.
  • Experience in data collection, entry and analysis required.
  • Experience with AHA/TJC indicators and standards.
  • Graduate from an approved nursing educational program as defined by the National Council of State Boards of Nursing (NCSBN).
Education/Experience/Licensure/Technical/Other:
  • Registered Nurse Licensure in the State of Florida.
  • Minimum 5 years' experience in Critical Care ED.
  • Knowledgeable in Word, Excel, and PP.
