Job Summary

SGA Youth and Family Services is seeking a highly skilled and compassionate Family Team Facilitator to join our team. As a key member of our staff, you will play a vital role in supporting youth and families who are dually-involved in the Child Welfare and Juvenile Justice systems.

Key Responsibilities
  • Collaborate with Youth and Families: Work closely with youth, families, and informal/formal supports to identify individualized needs and develop a plan of care to meet those needs.
  • Coordinate Services and Care: Display effective inter-personal skills, a dedication to helping youth and families reach their goals, and a strong attention to detail in the coordination of services and care.
  • Manage Caseload: Manage and maintain a maximum caseload of 15 families who are DCFS involved.
  • Develop and Implement Plans of Care: Collaborate with families to identify individualized needs and develop a plan of care to meet those needs, including assigning responsibilities to each team member to address needs.
  • Provide Consultation and Support: Provide consultation and support to the Family Team in the implementation of the plan of care and monitoring the progress toward goal achievement.
  • Advocate for Families: Advocate for the family, especially around meeting their needs in the least restrictive setting possible.
  • Complete Assessments and Reports: Complete the Child and Adolescent Needs and Strengths (CANS) Assessment tool at designated intervals and maintain all administrative tasks in a timely manner.
Requirements
  • Education: Bachelor's Degree required in the field of Counseling, Psychology, Criminal Justice, Social Work, Education or related program.
  • Experience: Experience coordinating care and/or working with youth and families on a direct service level, including basic knowledge of the Illinois child welfare system and local resources and service providers.
  • Skills: Strong collaborative, relationship building, and communication skills to effectively resolve problems, achieve goals, and meet deliverables, as well as proficiency in Microsoft Windows and internet proficiency, including the use of Google Drive applications.
  • Preferred Qualifications: Bi-lingual Spanish speakers, knowledge of the DCFS SACWIS data system, and the Child and Adolescent Needs and Strengths (CANS) Assessment.
Physical Requirements
  • Ability to Work Varying Hours: Ability to work varying hours, including evenings and weekends, to accommodate family's needs.
  • Transportation: Must have a vehicle with a valid driver's license and automobile insurance required.
  • Physical Demands: Must be able to remain stationary 50% of the time, frequently move inside the office, and occasionally ascend/descend stairs.
