Position Overview

Annual Salary: $45,000

The Family Services Team Leader works collaboratively with young individuals, their families, and both informal and formal support systems to assess unique needs for youth involved in both Child Welfare and Juvenile Justice systems. This role is crucial in developing a tailored care plan that addresses these needs effectively.

SGA Youth & Family Services is dedicated to providing a broad spectrum of social services within some of the most underserved communities, aiming for sustainable and impactful change.

Key Responsibilities:

  • Manage a caseload of up to 13 youth, aged 11-19, who are engaged with the DCFS and involved in the justice system.
  • Exhibit strong interpersonal skills, demonstrating a commitment to assisting youth and families in achieving their objectives while maintaining meticulous coordination of services.
  • Work in partnership with youth, families, and their support networks to pinpoint specific needs and formulate a comprehensive care plan.
  • Utilize effective communication skills to engage with youth, their support systems, child welfare caseworkers, and other service providers during initial and ongoing meetings.
  • Conduct thorough assessments across various life domains to identify strengths and areas for improvement.
  • Lead Family Team meetings, ensuring that the voices of youth and families are prioritized and respected.
  • Collaborate with the Family Team to create a detailed care plan, assigning roles and responsibilities to address both traditional and innovative service needs.
  • Guide the team in adapting the care plan to manage any potential crises.
  • Provide ongoing consultation and support to the Family Team throughout the implementation of the care plan, monitoring progress towards goals.
  • Advocate for youth and families, particularly in ensuring their needs are met in the least restrictive environments.
  • Maintain accurate administrative records through appropriate data management systems and case files.
  • Adhere to the principles of Wraparound facilitation as instructed during training.
  • Conduct home visits and community meetings as necessary.
  • Engage in continuous professional development to stay informed about relevant policies and practices.
  • Perform additional duties as assigned.

Qualifications:

  • Education: Bachelor’s Degree in Counseling, Criminal Justice, Social Work, or a related field is preferred.

Experience:

  • Experience in care coordination and direct service work with youth and families.
  • Familiarity with the Illinois child welfare and juvenile justice systems; knowledge of local resources is advantageous.
  • Experience working with diverse populations and in varied community settings.

Other Skills:

  • Strong collaborative and communication skills to effectively address challenges and meet objectives.
  • Awareness of cultural diversity and the ability to engage with various families and professionals.
  • Proactive approach in following up with youth and families to ensure timely completion of tasks.
  • Attention to detail and proficiency in using technology, including Google Drive applications.
  • Effective verbal and written communication abilities.
  • Ability to maintain confidentiality and exercise discretion.

Key Competencies:

  • Flexibility to work varied hours, including evenings and weekends, to meet family needs.
  • Possession of a vehicle, valid driver’s license, and automobile insurance is required.

Physical Requirements:

  • Ability to remain stationary for extended periods.
  • Frequent movement within the office and during program activities.
  • Occasional lifting of up to 25 lbs. as needed.
  • Ability to navigate outdoor conditions as required.
