Key Responsibilities- Collaborate with Youth and Families: Work closely with youth, families, and informal/formal supports to identify individualized needs and develop a plan of care to meet those needs.
- Facilitate Family Team Meetings: Lead and engage in effective family team meetings, ensuring the youth and family voice is heard and respected by all team members.
- Develop and Implement Plans of Care: Collaborate with the Family Team to develop a comprehensive plan of care, assigning responsibilities to each team member to address needs.
- Provide Consultation and Support: Offer consultation and support to the Family Team in the implementation of the plan of care and monitoring progress toward goal achievement.
- Advocate for Families: Advocate for the family, especially around meeting their needs in the least restrictive setting possible.
- Complete Administrative Tasks: Maintain all administrative tasks in a timely manner through Google Drive, Apricot online database, ETO case notes, and on-site case files.
- Education: Bachelor's Degree required in the field of Counseling, Psychology, Criminal Justice, Social Work, Education, or related program.
- Experience: Experience coordinating care and/or working with youth and families on a direct service level.
- Skills: Strong collaborative, relationship building, and communication skills to effectively resolve problems, achieve goals, and meet deliverables.
- Other: Knowledge of the DCFS SACWIS data system and the Child and Adolescent Needs and Strengths (CANS) Assessment is preferred.
- Must be able to remain stationary 50% of the time.
- Must be able to frequently move inside the office to access file cabinets, office machinery, etc.
- Must be able to transport oneself to and from home visits.
- Occasionally ascends/descends stairs.
- Constantly moves to assist during program activities.
- At times it may be required to lift up to 25 lbs. for various needs.
- Frequently moves in outdoor weather conditions.
