The Youth and Family Support Coordinator works collaboratively with adolescents, their families, and both informal and formal support systems to assess individual needs for youth involved in both Child Welfare and Juvenile Justice systems. The role requires strong interpersonal skills, a commitment to assisting youth and families in achieving their objectives, and meticulous attention to detail in service coordination.
SGA Youth and Family Services is dedicated to providing a comprehensive range of social services in some of the most underserved communities, fostering long-term community transformation.
PRIMARY RESPONSIBILITIES:
- Oversee and manage a caseload of up to 13 youths, aged 11-19, who are involved with both DCFS and the justice system.
- Exhibit strong interpersonal skills and a commitment to aiding youth and families in reaching their aspirations while ensuring precise coordination of services.
- Engage with youth, families, and their support networks to identify unique needs and formulate a tailored care plan.
- Utilize effective communication skills to facilitate initial meetings, youth gatherings, and ongoing family team discussions.
- Conduct comprehensive strength-based assessments across all life domains to identify both strengths and areas for improvement.
- Lead and facilitate Family Team meetings, ensuring that the voices of youth and families are prioritized and respected.
- Collaborate with the Family Team to create a detailed care plan, assigning roles and responsibilities to address identified needs, including both traditional and evidence-based interventions.
- Guide the team in adapting the care plan to manage any potential crises effectively.
- Provide ongoing consultation and support to the Family Team in executing the care plan and tracking progress towards established goals.
- Advocate for youth and families, particularly in securing services in the least restrictive environments.
- Maintain all administrative responsibilities promptly using Google Drive, ETO case notes, and on-site case files.
- Adhere to the principles of Wraparound facilitation as instructed during training.
- Conduct home visits and meet with families within their communities as necessary.
- Engage in continuous professional development and stay informed about DCFS policies.
- Perform other duties as assigned.
Education:
- Bachelor's Degree in Counseling, Criminal Justice, Social Work, or a related field is preferred.
- Experience in care coordination and direct service work with youth and families.
- Basic understanding of the Illinois child welfare and juvenile justice systems; familiarity with local resources is advantageous.
- Experience working with diverse populations and communities.
- Strong collaborative and communication skills to effectively resolve issues and achieve objectives.
- Awareness of cultural diversity and the ability to communicate effectively with various families and professionals.
- Proactive approach to follow up with disengaged youth and families to ensure timely task completion.
- Attention to detail and thoroughness in all tasks.
- Proficient in Microsoft Windows and internet applications, including Google Drive.
- Effective verbal and written communication abilities.
- Ability to maintain confidentiality and exercise discretion.
- Flexibility to work varying hours, including evenings and weekends, to meet family needs.
- Possession of a vehicle, valid driver's license, and automobile insurance is required.
- Ability to remain stationary for 50% of the time.
- Frequent movement within the office to access files and equipment.
- Capability to travel for home visits.
- Occasional stair navigation.
- Active participation in program activities.
- Occasional lifting of up to 25 lbs. as needed.
- Ability to work in various outdoor weather conditions.
