About the Role

Lutheran Social Services of Wisconsin and Upper Michigan is seeking a highly motivated and compassionate Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in connecting residents with resources in the community, providing monthly presentations on community issues, and connecting residents with recreational programming.

This is a 40-hour per week, benefit-eligible role that requires a strong understanding of supportive services and resources for older adults and individuals with disabilities. The ideal candidate will have a bachelor's degree in social work, sociology, psychology, or a related field, and at least 3 years of experience working in a social services setting serving older adults or individuals with disabilities.

Key Responsibilities
  • Provide general informal case management, including intake and referral services to all tenants needing such assistance.
  • Assess health, psychological, and social needs, and develop individually tailored case plans for services and periodic reassessments of the tenant's situation.
  • Establish linkages with all agencies and service providers in the community, and set up a directory of providers for use by both project staff and tenants.
  • Refer and link tenants to service providers in the general community, including case management, personal assistance, homemaker, meals on wheels, transportation, counseling, and legal advocacy.
  • Educate tenants on service availability, application procedures, client rights, and provide advocacy as appropriate.
  • Monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the individual.
  • May develop case plans in coordination with assessment services in the community, and provide training to project tenants on the obligation of tenancy or coordinate such training.
  • Presents workshops or informative sessions to tenants on subjects of tenant interest.
  • Demonstrate the ability to work with tenants, the agency, and the community in a positive manner, as well as the ability to work independently.
Requirements
  • Bachelor's degree in social work, sociology, psychology, or a related field.
  • At least 3 years of experience working in a social services setting serving older adults or individuals with disabilities.
  • Valid driver's license and reliable transportation.
  • Ability to communicate effectively in verbal and written format, and to read, write, analyze, and interpret to complete all required intervention and goal process documentation.
