Outpatient Care Coordinator II

We are seeking a highly skilled and compassionate Outpatient Care Coordinator II to join our team at Alta Hospitals. As a key member of our healthcare team, you will play a vital role in ensuring our patients receive seamless and coordinated care.
Key Responsibilities- Identify and Reduce Barriers to Care: Work closely with patients to identify and address any obstacles to their care, ensuring they receive the support they need to achieve optimal health outcomes.
- Coordinate Care Services: Collaborate with healthcare providers, specialists, and other members of the care team to ensure timely and effective coordination of care services.
- Provide Patient Education and Support: Educate patients on the importance of accessing care promptly and provide ongoing support to help them navigate the healthcare system.
- Manage Caseload and Prioritize Tasks: Efficiently manage and prioritize a caseload of patients, ensuring timely and effective care coordination.
- Communicate with Healthcare Providers: Serve as a liaison between patients, healthcare providers, and other members of the care team, ensuring seamless communication and coordination of care.
- Document Patient Information: Maintain accurate and up-to-date documentation of patient information, care plans, and interventions.
- Collaborate with Case Managers: Work closely with case managers to ensure optimal coordination of care and to identify opportunities for improvement.
- Identify Opportunities for Referral: Identify opportunities for referral to Long Term Support Services, Behavioral Health, and other community resources, ensuring patients receive comprehensive and coordinated care.
- Maintain Compliance with Departmental Guidelines: Ensure compliance with established departmental productivity guidelines and maintain accurate records and reports.
- Education: High School Diploma required; Associates Degree or higher preferred.
- Experience: Minimum of one (1) year of experience in Case Management Care Coordination in an HMO environment; or any combination of education and experience which would provide an equivalent background required.
- Certification/Licensure: Medical Assistant Certification preferred; Medical Terminology certificate preferred.
