About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our Emergency Department team at the University of California, Irvine. As a key member of our healthcare team, you will play a critical role in ensuring that patients receive comprehensive care and support during their visit to the emergency department and facilitate their journey through the healthcare system.

Key Responsibilities
  • Provide patient-centered care and support to individuals with HIV, HCV, Syphilis, Chlamydia, or Gonorrhea.
  • Work closely with nursing staff and physicians to assist in the treatment and follow-up on screening tests.
  • Consult with mental health and substance use advocates as necessary to provide holistic wrap-around care for patients.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare providers to ensure seamless transitions of care and coordinate follow-up appointments.
  • Provide education and support to patients and their families on disease management, treatment options, and available resources.
  • Maintain accurate and up-to-date patient records and communicate effectively with healthcare teams.
Requirements
  • Bachelor's degree in Nursing, Social Work, Public Health, or a related field.
  • Current state licensure or certification, where applicable.
  • 1-3 years of experience working in an emergency department, infectious disease care, or a similar healthcare setting.
Preferred Qualifications
  • A master's degree.
  • Experience working with diverse patient populations and in a fast-paced, dynamic environment.
