Qualifications:- A minimum of three (3) years of experience in an acute hospital environment.
- Expertise in areas such as Healthcare Management, Quality Improvement, Utilization Review, and Discharge Planning.
- Graduation from an Accredited Professional School of Nursing.
- Current RN license and American Heart Association (BLS) certification preferred.
The supervisor will facilitate and coordinate non-high-risk discharges, addressing age-related issues that may require intervention, particularly for geriatric patients.
Utilizing an interdisciplinary approach, the supervisor will develop plans and goals tailored to patient needs, ensuring effective discharge planning and coordination with Social Services for optimal care settings.
