Position Overview: Patient Access Specialist 1

CAN Community Health, a leading provider of HIV services in the nation, is seeking a dedicated Patient Access Specialist 1. This role is pivotal in our mission to support individuals affected by HIV and other infectious diseases. Key Responsibilities:

The Patient Access Specialist plays a crucial role in assessing, planning, and advocating for health services tailored to individual needs. This position emphasizes quality and cost-effective outcomes while managing the organization's 340B pharmacy options.

Career Development:

Patient Access Specialists operate at various levels, with potential for advancement based on clinic size, patient demographics, and performance. There are four levels: PAS I, PAS II, PAS III, and PAS Manager, with annual reviews to evaluate career progression.

Primary Duties:

1. Uphold the mission and values of CAN Community Health in all interactions.
2. Adhere to established policies and procedures.
3. Maintain confidentiality in accordance with professional standards.
4. Conduct assessments of Social Determinants of Health, documenting barriers and risks.
5. Facilitate access to 340B, RW, ACA, and other program services.
6. Identify patients at risk of disengagement from care.
7. Optimize patient communication across various platforms.
8. Conduct home visits for patients requiring additional support.
9. Ensure continuity of care through regular patient check-ins.
10. Educate clinics on the benefits of the 340B program.
11. Participate in training and meetings as required.
12. Serve as a liaison among patients, clinic staff, and community partners.
13. Discuss service costs and insurance options with patients.
14. Assist in medication prior authorization processes.
15. Ensure compliance with all regulatory requirements.

Additional Responsibilities:

1. Engage in quality improvement initiatives.
2. Utilize resources efficiently.
3. Pursue ongoing professional development in HIV-related knowledge.
4. Represent CAN Community Health at relevant conferences.

Qualifications:

Physical Requirements:
1. Ability to lift and move objects weighing up to 20 lbs.
2. Close visual acuity for data analysis and equipment operation.
3. Effective communication skills, both verbal and auditory.

Education and Experience:
1. A college degree or a minimum of three years of relevant experience.
2. Experience working with priority populations is preferred.

Preferred Qualifications:
1. Over three years of experience in HIV programmatic roles.
2. Background in nursing, medical assistance, or case management.
3. Bilingual candidates are encouraged to apply.

Skills and Abilities:
1. Strong teamwork and productivity focus.
2. Adaptability in a dynamic work environment.
3. Knowledge of HIV-related medical terminology and practices.
4. Proficient in Microsoft Office and other relevant software.
5. Strong organizational and problem-solving skills.

Travel Requirements:
Moderate travel within the state may be required, including community engagement during evenings and weekends.
