Job Title: Patient Care Coordinator

We are seeking a compassionate and knowledgeable Patient Care Coordinator to provide high-quality, individualized care to our patients. As a key member of our team, you will work collaboratively with patients to determine their medical needs, develop effective treatment plans, and oversee their care.

Responsibilities:
  • Develop and monitor patient care plans to ensure high-quality, individualized care.
  • Coordinate with healthcare professionals to ensure seamless care delivery.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and satisfaction.
  • Manage caseloads of 100+ patients, prioritizing care needs and ensuring timely interventions.
  • Provide education and support to patients on their healthcare options and match them with suitable caregivers.
  • Resolve grievances and complaints in a timely and professional manner.
  • Contribute to team efforts by accomplishing related results as needed.
Qualifications:
  • High school graduate or equivalent.
  • 3 years of experience in a Home Care or Service Coordination role.
  • Demonstrated capability to maintain strict confidentiality.
  • Proven ability to generate leads and monitor referrals.
  • Strong typing and computer skills.
  • Excellent communication, public relations, and follow-up skills.
