Job Summary

We are seeking a highly skilled Patient Navigator to join our Ambulatory Care team at Lancaster General. The successful candidate will be responsible for coordinating patient care, developing and implementing care plans, and providing advocacy to patients and their families.

Key Responsibilities
  • Coordinate patient care and develop and implement care plans in collaboration with the care team.
  • Provide advocacy to patients and their families, ensuring they receive the necessary support and resources.
  • Work closely with healthcare providers, social workers, and community resources to ensure seamless care coordination.
  • Conduct home visits and provide education to patients and their families on health literacy and medication adherence.
  • Enter and maintain accurate electronic records, compile reports, and complete other program documentation and administrative responsibilities in a timely manner.
  • Participate in interdisciplinary case conferences and team meetings to ensure effective care coordination.
Requirements
  • High School Diploma or General Equivalency Degree (GED) with at least one year of experience in the social services and/or medical field.
  • Strong understanding of medical terminology, case management, and outreach services.
  • Exceptional organizational and interpersonal skills, with attention to detail required; strong oral/written communication skills is a must.
  • Ability to work collaboratively in a team, manage multiple priorities, utilize effective time management skills, and exercise sound administrative and clinical judgment.
  • Demonstrated ability to work well with people of various ages, backgrounds, ethnicities, and life experiences.
  • Valid Pennsylvania Driver's License; must have daily access to an insured automobile.
  • Ability to travel to multiple office locations and into patient homes.
Preferred Qualifications
  • Licensed Practical Nurse (LPN), Paramedic/Emergency Medical Technician, or other licensed health care practitioner.
  • Bilingual.
  • Prior acute hospital experience or ambulatory practice experience.
  • Demonstrated comprehensive knowledge of social and community resources.
  • Prior experience with patient home visits.
Working Conditions

The successful candidate will work in a fast-paced, dynamic environment, requiring strong communication and interpersonal skills. The position involves frequent travel to multiple office locations and patient homes, and requires the ability to work independently and as part of a team.
