Job Summary

We are seeking a highly skilled Patient Navigator to join our Ambulatory Care Management team at Lancaster General. As a Patient Navigator, you will play a critical role in coordinating patient care, developing care plans, and providing advocacy to patients and their families.

Key Responsibilities
  • Coordinate patient care plans, including coordination of public assistance/entitlements, behavioral health services, and referrals to community resources.
  • Act as a liaison between healthcare providers, community resources, and managed care/insurance plans to ensure patient-centered coordination of care.
  • Engage patients and caregivers in health literacy coaching to achieve identified goals.
  • Forge partnerships with local service organizations and community resources to broaden support services for patients.
  • Enter and maintain electronic records, compile reports, and complete other program documentation and administrative responsibilities in a timely manner.
  • Participate in interdisciplinary case conferences/team meetings/huddles.
Requirements
  • High School Diploma or General Equivalency Degree (GED) with at least one year of experience in the social services and/or medical field.
  • Strong understanding of medical terminology, case management, and/or outreach services.
  • Exceptional organizational and interpersonal skills, with attention to detail required; strong oral/written communication skills is a must.
  • Ability to work collaboratively in a team, manage multiple priorities, utilize effective time management skills, and exercise sound administrative and clinical judgment.
  • Demonstrated ability to work well with people of various ages, backgrounds, ethnicities, and life experiences.
  • Valid Pennsylvania Driver's License; must have daily access to an insured automobile.
  • Ability to travel to multiple office locations and into patient homes.
Preferred Qualifications
  • Licensed Practical Nurse (LPN), Paramedic/Emergency Medical Technician, or other licensed health care practitioner.
  • Bilingual.
  • Prior acute hospital experience or ambulatory practice experience.
  • Demonstrated comprehensive knowledge of social and community resources.
  • Prior experience with patient home visits.
Working Conditions

This position requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The incumbent must be able to perform all job functions safely and efficiently.
