Role Purpose
Under the guidance of the Program Manager of Case Management Services, the Outreach Case Manager is responsible for engaging and assisting homeless individuals over the age of eighteen who are dealing with unresolved mental health and substance abuse challenges. The goal is to connect them with appropriate services that will help stabilize their lives, support their recovery, and empower them to overcome homelessness.

Key Responsibilities

  • Conduct outreach activities in various environments
  • Facilitate connections for clients needing social, mental health, educational, vocational, recreational, spiritual, or substance abuse services
  • Provide transportation assistance for clients to their appointments
  • Collaborate with other organizations and service providers
  • Engage in outreach efforts regardless of weather conditions
  • Participate in regular agency, program, and network meetings
  • Monitor assigned cases and complete all required documentation
  • Fill out First Contact Forms
  • Maintain and update weekly reports and client movement records
  • Accurately enter and manage client information in the Homeless Management Information System database
  • Perform other related duties to support a caseload of 17 clients

Qualifications

  • At least two years of experience in human services or a related field
  • Strong organizational and time management skills with a focus on meeting deadlines
  • Excellent written and verbal communication skills in English
  • Proficient in Microsoft Office and common desktop applications
  • Ability to effectively navigate the local network of substance abuse, mental health, and social service providers
  • Familiarity with the criminal justice system and the ability to engage and support individuals
  • Knowledge of mental health, substance abuse, dual diagnosis issues, and group dynamics
  • Comprehensive understanding of person-centered case management practices
  • Ability to meet all documentation requirements
  • Knowledgeable about crisis intervention, confidentiality, universal precautions, harm reduction, trauma-informed care, and motivational interviewing
  • Valid driver's license with a clean driving record

Supervision: Reports to the Program Manager of Case Management Services
Compensation: $25.50-$27.50 per hour based on education and experience, plus excellent benefits
