Role Purpose

The Permanent Supportive Housing Case Manager operates within the Permanent Supportive Housing Department, delivering case management services to adults experiencing homelessness who will reside in long-term supportive housing. This role involves assisting clients in securing and maintaining housing, while also connecting them to medical, mental health, substance abuse resources, entitlement/state benefits, and employment opportunities.


Key Responsibilities
  • Assist clients in locating, securing, and sustaining housing.
  • Link clients to resources aimed at preventing eviction and provide access to community services for social, mental health, educational, vocational, and substance abuse needs.
  • Communicate effectively with clients served.
  • Advocate for clients while promoting self-advocacy.
  • Establish and maintain professional boundaries while ensuring confidentiality.
  • Provide transportation to appointments on a case-by-case basis, as deemed appropriate.
  • Collaborate with other agencies and social service providers.
  • Build positive relationships with landlords and assist clients transitioning from supportive housing to community housing resources.
  • Embrace diverse backgrounds and deliver services with a non-judgmental approach.
  • Participate in weekly and monthly agency, program, and network meetings, and facilitate tenant meetings with fellow case managers.
  • Provide temporary coverage for vacant caseloads as needed.
  • Monitor assigned cases and fulfill all documentation requirements as mandated by Columbus House.
  • Conduct health and safety checks at least biannually.
  • Perform other related duties as assigned.

Qualifications
  • Strong organizational and time management skills with a focus on deadlines.
  • Excellent written and verbal communication skills in English.
  • Proficient in Microsoft Office and common desktop and online applications.
  • Knowledgeable in navigating substance abuse, mental health, and social services networks.
  • Familiar with criminal justice barriers and capable of engaging and motivating individuals served.
  • Understanding of mental health, substance abuse, dual diagnosis issues, and group dynamics.
  • Able to work with diverse populations, including youth, adults, and families.
  • Basic understanding of person-centered case management principles.
  • Ability to meet documentation requirements within established timeframes.
  • Knowledgeable in crisis intervention, confidentiality, universal precautions, harm reduction, and motivational interviewing.
  • Familiar with housing first principles, housing laws, and tenant rights/responsibilities.
  • Capable of fostering professional relationships with clients, external providers, landlords, and property managers.
  • Empathetic and compassionate towards individuals who have experienced homelessness and the factors contributing to it.
  • Valid driver's license with a clean driving record.

Supervision

Reports to: Scattered Sites PSH Manager


Work Schedule

32 hours per week; Monday-Thursday 8am-4pm
