Family Support Specialist

**Job Summary:**

We are seeking a highly motivated and experienced Family Support Specialist to join our team at East River Family Strengthening Collaborative, Inc. The successful candidate will be responsible for facilitating the transition of clients to independence through a strengths-based approach to social work practice.

**Key Responsibilities:**

* Conduct home visits to assess the needs of children and families, and provide case management and family strengthening services.

* Engage families and other supports to develop and implement goal-oriented interventions.

* Establish and maintain healthy, professional relationships to assist families in becoming independent of the child welfare system.

* Collect and document relevant information for reporting and case management purposes.

* Travel locally to conduct home visits and engage with community resources.

**Requirements:**

* 2-5 years of experience in a community-based human services field.

* Bachelor's degree in Social Work or related field.
