Role Summary

Key Responsibilities
  • Lead initiatives that enhance program structure and support all participants.
  • Empower youth and families through proactive engagement and support.
  • Maintain accurate and secure client documentation, including records and service plans.
  • Conduct regular supervision sessions with clients and their families.
  • Collaborate closely with the Program Manager for ongoing supervision and guidance.
  • Actively participate in weekly staff meetings, demonstrating leadership and engagement.
  • Assist clients in planning for discharge and aftercare as they transition from the program.
  • Provide timely and precise documentation of services, including progress reports and urgent issues.
  • Oversee client assessments and case management, ensuring individualized treatment plans are developed.
  • Perform additional duties as assigned.
Qualifications
  • Master's Degree in Social Work (MSW) is required.
  • 1-5 years of experience in Human Services, particularly with adolescents and young adults.
  • In-depth understanding of homelessness and related issues.
  • Proven ability to collaborate with community leaders to secure funding and lead complex projects.
