Job Overview

Position Summary:

The Community Support Specialist engages with individuals to foster optimal mental wellness and address behavioral health challenges within the service population. This role involves direct interaction with adolescents, adults, or family units to deliver personalized support in navigating issues related to mental and emotional well-being. The Community Support Specialist plays a crucial role in facilitating the client's journey towards independence, advocating for the use of community resources, and providing essential information and referral services, including mental health support.

Key Responsibilities:

  • Conducts necessary community support and care coordination, implementing therapeutic and skill-building activities as outlined in the client’s treatment plan.
  • Guides clients in developing skills and strategies to manage personal challenges, aiding them in their recovery and resilience-building efforts.
  • Maintains accurate and confidential treatment records and documentation promptly, ensuring compliance within 24 hours of service delivery.
  • Collaborates with colleagues and external partners to perform clinical assessments and formulate treatment plans that support client needs.
  • Facilitates individual and group sessions to assist clients in overcoming dependencies, adapting to life changes, and enhancing their skill sets.
  • Acts as an advocate for clients to coordinate necessary services and address urgent issues during crisis situations.
  • Engages with families, probation officers, law enforcement, and other stakeholders to share vital information throughout the treatment process.
  • Provides support to family members, helping them understand and assist clients or patients effectively.
  • Refers clients and family members to community resources or specialists as required.
  • Assesses the effectiveness of services and monitors client progress towards achieving set objectives.
  • Complies with all federal and local regulations governing behavioral health service delivery and adheres to the Hillcrest Compliance Plan and professional conduct standards.
  • May be required to work during weekends, holidays, and on-call hours as needed.
  • Performs additional duties as assigned.

Qualifications:

  • A Bachelor’s Degree (BA/BS) in a relevant field is preferred, or an equivalent combination of education and experience may be considered.
  • Experience with a credible system is highly desirable, along with proficiency in Microsoft Office applications.
  • Practical knowledge and skills in delivering a variety of therapeutic techniques and skill-building interventions.
  • Commitment to high standards of excellence.
  • Exceptional written and verbal communication abilities.
  • Strong organizational, analytical, and problem-solving skills.
  • Ability to prioritize tasks and manage workflow effectively.
  • Flexibility and adaptability to work within a dynamic environment.
  • Capability to work independently as well as collaboratively within teams and committees.
  • Excellent interpersonal skills.
  • Sound judgment with the ability to make timely and informed decisions.
  • Creative and innovative team player.
  • Bilingual skills are a plus.
  • Demonstrated competence in managing emergencies effectively.
  • Able to collaborate with various health professionals in a busy and complex setting with tact and professionalism.
  • Must meet established productivity standards.
  • Strong documentation skills using electronic medical records.
  • Ability to communicate effectively with individuals from diverse backgrounds.
