Position Summary:

This role involves direct engagement with adolescents, adults, or family units to deliver tailored support in navigating mental and emotional health issues. The Community Support Specialist plays a crucial role in facilitating clients' journey towards independence, promoting the use of community resources, and providing essential information and referral services related to mental health.

Key Responsibilities:

  • Delivers essential community support, care coordination, and therapeutic activities as outlined in the client’s treatment plan.
  • Empowers clients to develop skills and strategies for managing personal challenges, fostering recovery and resilience.
  • Maintains accurate and confidential treatment records and documentation promptly, ensuring compliance with service delivery timelines.
  • Collaborates with internal staff and external partners to conduct clinical assessments and formulate effective treatment plans.
  • Facilitates individual and group sessions to assist clients in overcoming dependencies and adapting to life changes through skill enhancement.
  • Acts as an advocate for clients, coordinating necessary services and addressing urgent issues during crises.
  • Engages with families, probation officers, law enforcement, and other stakeholders to share vital information throughout the treatment process.
  • Provides support to family members, helping them understand and assist clients effectively.
  • Refers clients and families to community resources or specialists as needed.
  • Assesses the effectiveness of services and monitors clients' progress towards their goals.
  • Complies with all applicable federal and local regulations governing behavioral health services and adheres to the organization's compliance standards.
  • May be required to work on weekends, holidays, and during on-call hours.
  • Performs additional duties as assigned.

Qualifications:

  • A Bachelor’s Degree (BA/BS) in a relevant field is preferred, or an equivalent combination of education and experience.
  • Experience with credible systems is highly desirable, along with proficiency in Microsoft Office applications.
  • Practical knowledge and skills in delivering various therapeutic techniques and skill-building interventions.
  • Commitment to high standards of excellence.
  • Exceptional written and verbal communication abilities.
  • Strong organizational, analytical, and problem-solving skills.
  • Ability to prioritize tasks and manage workflow effectively.
  • Flexibility and willingness to adapt to changing priorities with enthusiasm.
