East River Family Strengthening Collaborative, Inc. is seeking a dedicated professional to join our team as a Family Support Specialist. In this role, you will play a critical part in facilitating a client's transition to independence through the use of a strengths-based approach to social work practice. Your expertise will be instrumental in endorsing and applying community resources, providing information and referral services, and advocating for clients in areas such as education, childcare, housing, employment, and court and public systems.
Key Responsibilities:
* Conduct home visits to assess the conditions, level of functioning, well-being, and risk of children and families, and identify services needed.
* Engage families and other supports to assess their needs and implement goal-oriented interventions.
* Establish and maintain healthy, professional relationships to assist families in becoming independent of the child welfare system.
* Collect and document relevant information for reporting and case management purposes.
* Local travel is required.
Requirements:
* Two to five years of experience in a community-based human services field or similar capacity.
* Bachelor's degree in Social Work or a related field is desired.
