Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at the Mission Neighborhood Health Center. As a Case Manager, you will play a critical role in providing on-going case management and crisis counseling to our participants, while also connecting them with our treatment team and San Francisco medical and behavioral health system programs.

Key Responsibilities
  1. Assessment and Planning: Perform individual comprehensive assessments of our participants' psychosocial history, mental health status, medical needs, housing, food, and other basic support needs. Develop care plans that outline steps necessary to accomplish activities, goals, and objectives.
  2. Intervention and Coordination: Implement care plans through the provision of interventions, coordination with our integrated services team, and facilitated referrals to community resources.
  3. Supportive Services: Utilize a participant-centered, harm reduction model to provide supportive services to individuals, families, and groups of patients. This includes motivational interviewing, cognitive behavioral, and health psychology treatment methods.
  4. Referrals and Linkages: Refer clients to appropriate shelter case management beds and assist them in finding, applying for, accessing, and stabilizing in housing. Assist clients in compiling documentation and filling out forms for immigration and public benefits' eligibility.
  5. Documentation and Reporting: Receive and document monthly locker fees according to protocol. Maintain accurate and timely records of client interactions, progress, and outcomes.
Requirements
  1. Education and Experience: Bachelor's Degree in Social Work, Psychology, Counseling, or a related field, and at least two years of paid work experience with homeless adults with multiple disabilities. Masters' Degree in Social Work (MSW) and/or Masters in Family and Child Therapist (MFT) degree preferred.
  2. Language Skills: Bilingual Spanish/English required.
  3. Knowledge and Skills: Knowledge of motivational interviewing, cognitive behavioral, and health psychology. Ability to work with de-escalation techniques, conflict resolution, and emergency response. Excellent interpersonal communication and writing skills.
  4. Professional Qualities: Ability to work professionally and ethically in a multi-cultural team setting, including the ability to maintain a high level of confidentiality. Ability to work a flexible schedule, including evenings.
