Homeless Services Case Manager

PATH is seeking a dedicated and compassionate Case Manager to join our Care Coordination Services team. As a Case Manager, you will play a vital role in assisting clients in breaking the cycle of homelessness by providing individualized supportive services.
Key Responsibilities:
- Conduct intake and individualized needs assessments for clients, collaborating to develop a personalized plan that addresses housing goals, income, and other personal objectives.
- Provide referrals, linkages, information, and support to resources that help clients achieve their goals.
- Complete program-specific assessments, such as the VI-SPDAT, to evaluate strengths and challenges.
- Evaluate and document client progress, conducting regular reviews to ensure goals are being met.
Requirements:
- 2+ years of experience in healthcare, case management, homeless services, or a related field.
- Excellent analytical, communication, and problem-solving skills.
- Ability to work effectively with diverse populations and maintain professional boundaries.
