This position is dedicated to enhancing patient care through effective community support and resource mobilization. The Community Care Coordination Specialist plays a crucial role in ensuring that patients and their families navigate complex psychosocial and economic challenges, ultimately striving for the best possible health outcomes.

The primary responsibilities include:

  • Identifying individuals and families requiring ongoing care coordination or community assistance
  • Collaborating with multidisciplinary teams to formulate safe and efficient discharge strategies
  • Facilitating post-acute transitions for patients with intricate needs in partnership with care management professionals
  • Leading discussions with families to address advance care planning, comfort measures, and other essential care considerations
  • Advocating for effective discharge processes and comprehensive patient care

Minimum Qualifications:
Education: A Master's degree in Social Work is essential.
Experience: A minimum of one year in discharge planning within an acute or subacute healthcare environment is required.
Licensure: LMSW/LCSW credentials are preferred. ACMA certification is also desirable.

Physical Requirements:
This role necessitates prolonged periods of standing, walking, bending, and lifting. Candidates must possess strong communication skills, as well as auditory and visual acuity. Attention to detail is crucial, along with the ability to lift or move weights ranging from 20 to 50 lbs. There may be exposure to various chemicals, bodily fluids, and infectious agents.
