Client Services Coordinator

The role of the Service Coordinator is vital in ensuring that residents receive the necessary support and resources to enhance their quality of life. This position involves a variety of responsibilities aimed at assessing and addressing the needs of our residents.
Key Responsibilities- Assessment and Evaluation: Conduct thorough evaluations within the initial weeks of a resident's stay to assess their ability to perform Activities of Daily Living and to understand their personal interests.
- Service Coordination: Organize essential services such as transportation, personal care, and home-delivered meals to meet residents' needs.
- Ongoing Monitoring: Perform annual reassessments to track changes in residents' functioning and document these changes accurately.
- Family Engagement: Collaborate with family members and residents to address their needs effectively.
- Resource Referral: Provide information and referrals to government-supported services, including Medicare/Medicaid benefits and food assistance programs.
- Advocacy: Act as an advocate for residents when necessary, ensuring their voices are heard.
- Compliance and Training: Complete required training and adhere to corporate compliance standards, including safety policies and HIPAA regulations.
- Documentation: Maintain accurate and timely records of all interactions and assessments.
To be successful in this role, candidates should possess:
- A Bachelor’s degree in social work, gerontology, or a related field.
- At least two years of relevant experience.
- Bilingual proficiency in Russian and English is preferred for specific positions.

- Ability to effectively present information and respond to inquiries from various stakeholders.
