Position Overview:

The role of the Service Coordinator is pivotal in ensuring that residents receive the necessary support to thrive in their living environment. This position involves a variety of essential duties aimed at enhancing the quality of life for our clients.

Key Responsibilities:
  • Conduct initial assessments within the first two weeks of a new resident's stay to evaluate their capacity to manage Activities of Daily Living and to identify their personal interests.
  • Coordinate additional services as required, such as homemaking, transportation, meal delivery, and personal care assistance.
  • Perform annual reassessments to monitor changes in residents' needs and document any alterations in their functioning.
  • Collaborate with family members and residents to address their support needs effectively.
  • Engage with discharge planners and rehabilitation services for residents transitioning from hospital care.
  • Provide residents with comprehensive information and referrals to government-supported services, including Nursing Home Services, Medicare/Medicaid Benefits, and other assistance programs.
  • Act as an advocate for residents when necessary.
Compliance and Training:

Maintain corporate compliance by completing 36 hours of approved training within the first year and 12 hours annually thereafter. Adhere to safety policies and procedures, and comply with laws regarding the protection and confidentiality of client information, including HIPAA regulations.

Qualifications:
  • A Bachelor's degree in social work, gerontology, or a related field is required.
  • At least two years of relevant experience is preferred.
  • Bilingual proficiency in Russian and English is advantageous for certain positions.
Skills:

The ideal candidate will possess strong analytical and communication skills, with the ability to interpret business documents and governmental regulations. They should be capable of writing reports and presenting information effectively to various stakeholders.
