Position Summary: The Healthcare Coordinator will oversee medical nursing services, manage healthcare operations, and ensure accurate medical documentation while supervising assigned residential nursing personnel.

Work Schedule: Monday - Friday, 8:00 AM - 4:00 PM (40 hours)

Key Responsibilities:

  1. Supervise daily nursing operations within the designated area.
  2. Evaluate the needs of individuals served by Bridgewell and collaborate with nursing staff to develop appropriate care plans.
  3. Manage the clinical nursing requirements of individuals by coordinating with nursing team members, physicians, visiting nurse associations, and program staff.
  4. Support professional nursing staff to ensure proficiency with the DPH MAP system, clinical training, and medical matters.
  5. Facilitate regular communication among program directors, supervisors, and nursing staff.
  6. Provide oversight to nursing team members through monthly residential meetings and daily support for clinical inquiries.
  7. Maintain communication with the Director of Health Care by participating in weekly supervision and submitting a weekly written report.
  8. Contribute to the staff evaluation process by providing input and assisting in preparation.
  9. Assist in the recruitment and interview process for new nursing personnel.
  10. Manage triage nursing team calls for clinical support to assigned nursing staff.
  11. Attend relevant meetings concerning changes in healthcare status following hospital discharge, LTCF, or other medically necessary situations.
  12. Oversee the transition and placement of new referrals into existing or newly established Bridgewell programs.
  13. Perform additional duties as assigned.

Qualifications:

  1. Associate's or Bachelor's Degree and a current Registered Nurse license with the Massachusetts Board of Registration in Nursing.
  2. Minimum of one year of experience working with individuals with disabilities.
  3. At least one year of experience in a community setting.

Skills and Knowledge:

  1. Familiarity with DDS Regulations and CARF standards.
  2. Understanding of behavior management and human rights.
  3. Strong organizational, problem-solving, and communication skills.

  1. Frequent climbing of stairs is required.
  2. Ability to lift and/or move up to 20 pounds.
  3. Must possess a valid driver's license, with normal eyesight and hearing capabilities, with or without corrective measures.
