Position Summary: The Healthcare Coordinator will manage medical nursing services, oversee healthcare operations, and ensure accurate medical documentation while supervising assigned residential nursing personnel.
Work Schedule: Monday - Friday, 8:00 AM - 4:00 PM (40 hours per week)
Key Responsibilities:
- Supervise daily nursing operations within the designated area.
- Evaluate the needs of individuals served by Bridgewell and develop appropriate care plans in collaboration with nursing staff.
- Oversee the clinical nursing requirements of individuals by coordinating with nursing team members, physicians, visiting nurse associations, and program staff.
- Support professional nursing staff to ensure proficiency with the DPH MAP system, clinical training, and medical concerns.
- Facilitate regular communication among program directors, supervisors, and nursing personnel.
- Provide oversight to nursing staff through monthly residential meetings and daily support for clinical inquiries.
- Communicate with the Director of Health Care by participating in weekly supervision and submitting a weekly written report.
- Assist in the evaluation process for nursing staff members.
- Participate in the recruitment and interview process for new nursing personnel.
- Manage triage nursing team calls for clinical support to assigned nursing staff.
- Attend relevant meetings regarding changes in healthcare status following hospital discharge, LTCF, or other medically necessary situations.
- Oversee the transition and placement of new referrals into existing or newly established Bridgewell programs.
- Perform additional duties as assigned.
Qualifications:
- Associate's or Bachelor's Degree and a current Registered Nurse license with the Massachusetts Board of Registration in Nursing.
- Minimum of one year of experience working with individuals with disabilities.
- At least one year of experience in a community-based setting.
Skills and Knowledge:
- Familiarity with DDS Regulations and CARF standards.
- Understanding of behavior management and human rights.
- Strong organizational, problem-solving, and communication skills.

- The employee must be able to lift and/or move up to 20 pounds.
