As a vital member of our organization, the Case Manager plays a crucial role in supporting clients through comprehensive assessments and tailored service plans.
Key Responsibilities:
1. Conduct thorough evaluations of client needs and document findings effectively.
2. Collaborate in the creation of personalized service plans that align with the client's assessment.
3. Facilitate the provision of essential services identified during assessments.
4. Aid clients in accessing necessary services through referrals and connections.
5. Track and monitor client progress by coordinating appointments, following up on services, and reassessing needs periodically.
6. Advocate for clients by engaging in activities such as writing letters to expedite benefits and empowering clients and their families to advocate for themselves.
7. Maintain accurate case records that document interactions, required services, reports, and client progress, adhering to regulatory standards.
8. Provide case consultation by collaborating with service providers to assess client status and progress.
9. Communicate effectively with supervisors regarding case management issues, resource allocation, and service collaboration.
10. Deliver psychosocial education to clients and families to enhance their understanding of health conditions and social circumstances.
11. Offer crisis intervention services, including emergency referrals and coordination of urgent support.
12. May take on the role of Primary Case Coordinator, with responsibilities varying based on experience and skill level.
Qualifications:
• A Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field recognized by Medicaid or relevant funding sources.
• Completion of state-mandated case management/service coordination training within six months of employment.
Experience:
ADULT DIVISION: Minimum of one (1) year of full-time experience post-Bachelor's degree working directly with adults or the ACT population in a human services environment. A relevant Master's degree may substitute for experience.
