The Community Engagement Specialist collaborates with site leadership, patient relations supervisors, and marketing teams to pinpoint opportunities and develop as well as execute growth and retention strategies. This role focuses on converting prospective patients into our managed care plans. Acts as the primary contact for both potential and current patients, while also supporting external partners such as agencies and community organizations. Serves as the expert to assist patients with non-medical inquiries and navigates them through our healthcare system to foster a positive patient experience.

  • Oversee the growth and retention of site-level membership; implement strategies related to enrollment periods and beneficiary conversions.
  • Manage and conduct targeted outreach while tracking outcomes related to enrollment periods and conversions to managed care plans.
  • Coordinate with front office and care teams to facilitate new patient orientations and enhance awareness of available services.
  • Conduct new patient onboarding, providing comprehensive tours of the facility and detailed explanations of benefits and guidelines.
  • Lead service recovery initiatives at the site in collaboration with patient relations supervisors and site management.
  • Utilize data analytics and reporting to monitor progress against established growth and retention objectives.
  • Drive onboarding of new providers and support membership retention efforts from a marketing perspective.
  • Provide insightful feedback to the growth team and patient relations supervisors regarding broker engagement and performance, recommending areas for improvement.
  • Document detailed outcomes of patient outreach and retention activities in relevant systems.
  • Implement best practices for enhancing patient experience and collaborate with site teams to maximize retention.
  • Conduct outreach for new, transitioned, and termed patients.
  • Support provider transition plans and membership retention initiatives.
  • Coordinate patient retention efforts related to mergers and acquisitions.
  • Offer training on enrollment processes and site-specific initiatives impacting operations.
  • Collaborate with growth teams to plan and organize retention events and represent the organization at external marketing events as needed.
  • Partner with the growth team to identify opportunities and maintain relationships with agencies and community organizations to achieve growth targets.
  • Ensure the protection and confidentiality of patient information in compliance with HIPAA standards.
  • Perform additional duties as assigned.

Required Qualifications:

  • High school diploma or equivalent.
  • 1+ years of experience in a healthcare environment.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Excellent communication and customer service skills.

Preferred Qualifications:

  • Experience in managed care or ambulatory care settings.
  • Background in a sales or marketing role with performance targets.
  • Customer service experience.
  • Familiarity with health plans.
  • Bilingual in English and Spanish.
