Job Summary

We are seeking a highly skilled and compassionate Health/Human Service Case Manager I to join our team at the Department of Health and Social Services (DHSS). As a key member of our team, you will play a vital role in determining initial and continued eligibility for various benefits and services, including food assistance, medical assistance, and temporary assistance to needy families.

Key Responsibilities
  • Conduct thorough interviews with clients, family members, and other stakeholders to assess eligibility and connect clients with necessary health and human services.
  • Develop and implement service plans to aid clients in receiving care and assistance, collaborating with hospitals, clinics, and other resources as needed.
  • Evaluate information from client interviews, case history, and other sources, referring to proper channels for review and approval.
  • Provide guidance and relevant information to clients and families prior to receiving services and assistance.
  • Coordinate with other agencies, hospitals, and resources to expedite the process on behalf of the client.
  • Prepare case histories and reports, maintaining accurate records and chronological logs of case activities.
  • Utilize automated information systems and computer software to research, review, evaluate, and report on health and human service data.
  • Communicate effectively with people from diverse backgrounds, cultures, and abilities.
Requirements
  • Possession of an Associate's degree or higher in Behavioral or Social Science or a related field.
  • Three years of experience in health or human services support, including interviewing clients and assessing personal, health, social, or financial needs.
  • Six months of experience in making recommendations as part of a client's service plan.
  • Six months of experience in using an automated information system to enter, update, modify, delete, retrieve, and report on data.
  • Six months of experience in narrative report writing.
