About the Role
We are seeking a skilled Health/Human Service Case Manager II to join our team at the Department of Health and Social Services (DHSS). As a key member of our team, you will play a critical role in determining initial and continued eligibility for various needs-based benefits, including Food Supplement Benefits, Medical Assistance, and Temporary Assistance to Needy Families.
Key Responsibilities
- Interview clients, family members, and professional sources to assess their needs for health and human services.
- Develop comprehensive service plans utilizing in-house resources, referrals, and contracted providers.
- Conduct ongoing monitoring and assessment of service delivery to ensure quality and efficiency.
- Maintain accurate and detailed case records, summaries, and statistical reports.
- Establish effective working relationships with clients, families, and community partners.
Requirements
- 6 months of experience in case management, including assessing, planning, and implementing services to meet individual needs.
- 6 months of experience in making recommendations for client service plans, such as clinical treatment or determining eligibility for health or human services.
- 6 months of experience in interpreting laws, rules, and regulations.
- 6 months of experience in using automated information systems to enter, update, and retrieve data.
