Join Our Team as a Hospice RN Case Manager

At Interim HealthCare Home Health and Hospice, we are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a Case Manager. As a Hospice RN Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities:

  • Conduct ongoing assessments of patients' physical and psycho-social status
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families
  • Provide education and interventions to support effective pain and symptom management
  • Ensure timely and accurate documentation of patient services
  • Participate in IDG meetings and contribute to the development of patient care plans

Requirements:

  • Active, unrestricted RN license in the State of Michigan
  • At least one year of relevant experience as an RN
  • One year of management experience (preferred)
  • Current CPR Certification
  • Reliable transportation with a valid state driver's license and current car insurance
