About the Role

We are seeking a skilled Health/Human Service Case Manager II to join our team at the Department of Health and Social Services (DHSS). As a key member of our team, you will play a critical role in determining initial and continued eligibility for various needs-based benefits, including Food Supplement Benefits, Medical Assistance, and Temporary Assistance to Needy Families.

Key Responsibilities

  • Conduct interviews with clients, family members, and professional sources to assess their needs for health and human services.
  • Develop comprehensive service plans that utilize in-house sources, referrals, and contracted providers to meet client needs.
  • Monitor and assess service delivery to ensure optimum quality and efficiency, authorizing and recommending necessary changes and adjustments.
  • Maintain accurate and detailed case records, summaries, and statistical reports.
  • Establish effective working relationships with clients, family members, community organizations, and professional sources.

Requirements

  • 6 months of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
  • 6 months of experience in making recommendations as part of a client's service plan, such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
  • 6 months of experience in interpreting laws, rules, regulations, standards, policies, and procedures.
  • 6 months of experience in using an automated information system to enter, update, modify, delete, retrieve/inquire, and report on data.
  • 6 months of experience in narrative report writing.
