About the Role
We are seeking a skilled Health/Human Service Case Manager I to join our team at DHSS. In this role, you will play a critical part in determining initial and continued eligibility for various benefits, including Food Supplement Benefits, Medical Assistance, and Temporary Assistance to Needy Families.
Key Responsibilities
- Interview clients, family members, and others to assess eligibility and connect them with health and human services.
- Develop service plans to aid clients in receiving necessary care and assistance, collaborating with hospitals, clinics, and other resources.
- Evaluate client information and refer cases to proper channels for review.
- Provide guidance and information to clients and families prior to receiving services.
- Coordinate with other agencies and resources to expedite the process on behalf of the client.
- Prepare case histories and reports, maintaining records and chronological logs of case activities.
- Utilize automated information systems and computer software to research, review, and report on health/human service data.
- Communicate effectively with people across cultures, abilities, genders, ethnicities, ages, and races.
Requirements
- Possession of an Associate's degree or higher in Behavioral or Social Science or a related field.
- Three years' experience in health or human services support, including interviewing clients and assessing needs.
- Six months' experience in making recommendations as part of a client's service plan.
- Six months' experience in using automated information systems to enter, update, and retrieve data.
- Six months' experience in narrative report writing.
