Position Overview

The individual in this role will assess initial and ongoing eligibility for various assistance programs managed by the Division, which includes benefits such as Food Supplement Benefits (SNAP), Medical Assistance, and Temporary Assistance to Needy Families (TANF).

Preferred Qualifications

Candidates lacking the preferred qualifications may still be considered if they meet the job requirements. Proficiency in both Spanish and English, both written and spoken.

Key Responsibilities

The key responsibilities are essential functions common to all roles within this classification and are not intended to be an exhaustive list of all duties for any one position. This role is distinct from that of a licensed Social Worker, as it does not involve diagnosing mental or emotional disorders or providing therapeutic treatment. The employee may collaborate as part of a multi-disciplinary team to implement health services or treatment plans in accordance with applicable regulations. Conducts interviews with clients, family members, and other stakeholders to assess eligibility and connect clients with necessary health and human services. Develops a comprehensive service plan to assist clients in obtaining required care and support, collaborating with hospitals, clinics, and other relevant resources (Service plans may encompass job training, housing assistance, in-home services, and skill development). Analyzes information gathered from client interviews and case histories, referring to appropriate channels for further review. Provides essential guidance and information to clients and families prior to the receipt of services. Coordinates with various agencies and resources to streamline processes on behalf of clients. Prepares detailed case histories and reports, maintaining accurate records and logs of case activities. Utilizes automated systems and software to research, evaluate, and report on health and human service data. Engages effectively with diverse populations across various cultures, abilities, genders, ethnicities, ages, and races. Performs additional related duties as necessary.

Job Requirements

Qualifications for Community Support Case Manager I
Candidates must demonstrate competence in the following areas:

An Associate's degree or higher in Behavioral or Social Science or a related field.

OR

Three years of experience in health or human services support, including client interviews and assessing personal, health, social, or financial needs. Six months of experience in making recommendations as part of a client's service plan. Six months of experience using automated information systems for data management. Six months of experience in narrative report writing.
