Role Overview:
The Case Manager plays a crucial role in delivering case management and supportive services to unaccompanied immigrant minors released from federal custody.

Core Responsibilities:
  • Conduct home visits and phone consultations to evaluate the safety and suitability of the child's placement with their caregiver (sponsor) while linking the child to community services to mitigate risk factors.
  • Provide ongoing case management with an emphasis on screening and making suitable referrals to meet the child's needs across various domains, including placement, safety, education, health, mental health, legal/immigration matters, substance abuse, independent living, and enrichment activities.
  • Deliver crisis intervention services directly when necessary or connect families to appropriate resources to ensure the minor's safety and well-being during emergencies.
  • Prepare detailed documentation in accordance with LIRS policies and procedures.
  • Apply best practices in social work to conduct thorough assessments of the child's progress and the services provided.
  • Maintain consistent and timely communication with supervisors regarding any concerns related to the child's safety, stability, or well-being.
  • Engage in ongoing supervision and professional development.
  • Perform additional duties as assigned.
Qualifications:
  • Participation in all mandatory organizational training sessions.
  • Attendance at departmental and program meetings to stay informed about policy changes and best practices.
  • Engagement in training opportunities that promote professional growth in case management and documentation.
  • Willingness to travel for training, conferences, or to transport youth as required.
  • Commitment to maintaining a safe and clean work environment.
  • Assist the Program Manager with internal audits and various projects.
Physical Demands:
  • Must successfully pass an FBI background check and a CA/N check for all states of residence in the past five years.
  • Able to travel to designated counties as specified in the contract, potentially up to 70% of the time.
  • Capable of sitting and standing for extended periods while utilizing a telephone, computer, and other office equipment.
  • Ability to thrive in a fast-paced work environment.
Education:
Bachelor's degree in behavioral sciences, human services, or a related social services field.

Experience:
  • Experience in child welfare and/or case management is highly desirable.
  • A minimum of three years of experience working with refugee or immigrant children is preferred.
  • Cross-cultural or international experience is advantageous.
Skills:
  • Proficient in computer applications, particularly Microsoft Windows (Word, Excel).
  • Possession of a valid state driver's license and eligibility to drive as required by program services.
  • Flexibility in scheduling to accommodate program needs.
  • Knowledge of community resources and the ability to connect families with necessary services.
  • Strong clinical and crisis intervention skills.
  • Excellent verbal and written communication abilities.
  • Critical assessment and analytical skills.
  • Bilingual proficiency in English and Spanish is required.

  • Reports directly to the Program Manager.
  • Ability to work independently with minimal supervision.
  • Collaborative team player.
  • Meets monthly performance goals.
  • Ensures accurate, complete, and timely submission of all required documentation.
