Case Manager RN

Job Overview:
The Case Manager RN plays a crucial role in managing patient care and ensuring effective transition planning. This position involves working closely with a multidisciplinary team to establish care plans, treatment protocols, anticipated Length of Stay, and discharge strategies. The Case Manager RN reviews patient records to assess clinical stability and actively participates in rounds to track patient progress. They identify and address any obstacles to the care plan and collaborate with social services to address patients' psychosocial requirements. Accurate data entry, planning for medical discharge needs, and executing tailored action plans for patients are essential responsibilities of this role. A valid Registered Nurse license and a minimum of two years of experience in Case Management are required.

Key Responsibilities:
  • Formulate and execute discharge strategies
  • Incorporate the patient's narrative into the care plan
  • Facilitate access to services to promote positive patient outcomes
  • Work with various departments to resolve care barriers
  • Lead Care Coordination Rounds and engage in quality improvement initiatives
  • Foster collaborative relationships advocating for patient needs
  • Conduct team discussions regarding unplanned readmissions
  • Complete necessary documentation for patient transitions
