Position Summary:

This role serves as the initial point of contact for patients via phone, MyChart, and other communication channels. You will assist patients in scheduling appointments, processing medication refills, and providing information on operational support services and clinical resources. Delivering exceptional customer service in alignment with Confluence Health's vision and standards is essential.

Reporting Structure:
This position reports to the Clinic Patient Access & SWBD Supervisor.

Key Responsibilities:
  1. Professionally handle inbound calls promptly and direct them as needed.
  2. Manage appointment scheduling and cancellations, ensuring clear communication of details and necessary preparations.
  3. Verify appointment specifics, including date, time, location, insurance, and provider information. Update patient records as necessary.
  4. Act as a liaison between patients and their care teams.
  5. Facilitate medication refills by assessing prescription needs and coordinating with pharmacies.
  6. Employ de-escalation techniques when necessary.
  7. Document and relay patient-related messages accurately within the patient’s chart.
  8. Identify urgent situations and coordinate with healthcare providers as needed.
  9. Assist patients in navigating the Confluence Health system and connect them with appropriate resources.
  10. May assist with switchboard duties as required.
  11. Perform additional tasks as assigned.

Qualifications:

Required:

  • High School diploma or equivalent (GED).
  • Experience in healthcare, call center, or customer service settings.
  • Ability to thrive in a dynamic environment with strong customer service skills.
  • Excellent communication and data entry skills.

Desired:

  • Experience with computerized systems.
  • Familiarity with medical terminology.
