Role Summary:

The Permanent Supportive Housing Case Manager (PSHCM) plays a crucial role in delivering housing and support services to families and individuals facing chronic homelessness within the community. Utilizing the Housing First model, this position emphasizes the swift provision of housing without prerequisites, ensuring that vulnerable populations receive the assistance they need promptly.

Key Responsibilities:
  • Facilitate comprehensive client support and engagement initiatives to address both immediate and long-term needs in a compassionate and structured manner.
  • Promote outreach efforts to connect families and individuals with their permanent supportive housing requirements through collaboration with the Department of Human Services.
  • Assist clients in navigating housing applications and lease agreements with landlords and relevant contractors.
  • Conduct initial assessments of housing units and support clients during inspections to ensure compliance with program standards.
  • Collaborate with clients to formulate service plans, establish goals, and implement strategies to achieve these objectives, while regularly tracking and reporting progress.
  • Perform ongoing home visits to evaluate client needs and provide appropriate referrals.
  • Engage with community service providers and partner organizations to identify resources that can assist families.
  • Maintain a minimum of four face-to-face interactions with clients each month.
  • Document client information in Housing and Homeless (HTH) databases, generate reports, and process financial assistance checks.
  • Organize and lead weekly meetings based on client assessments.
  • Keep detailed and organized records for each client in accordance with DHS standards.
  • Communicate any issues related to case management to the Team Lead and Program Manager.
  • Compile consumer data for weekly and monthly reporting.
  • Participate in all mandatory training sessions and supervision meetings.
  • Assist clients with Activities of Daily Living (ADL) as necessary.
  • Perform other relevant duties as assigned.
Qualifications:
  • Two to six years of experience in the homeless services sector (required).
  • Bachelor's Degree.
  • Experience in crisis management (preferred).
  • Strong written and verbal communication skills.
  • Excellent time management and organizational abilities.
  • Proven case management skills with a successful track record.
  • Understanding of the factors contributing to homelessness.
  • Experience collaborating with landlords and knowledge of local rental markets.
  • Able to thrive in a team-oriented environment.
  • Skilled in serving a diverse population.
Additional Information:

Salary Range: $52,000 - $55,000
