Position Overview:

Benchmark Human Services is a leading organization dedicated to supporting individuals with intellectual and developmental disabilities (IDD) and behavioral health challenges. We are currently seeking a Registered Nurse (RN) Healthcare Coordinator to provide essential healthcare coordination and direct nursing support to individuals facing crises or at risk of significant challenges.

Key Responsibilities:

  • Coordinate healthcare services for individuals in compliance with all applicable regulations.
  • Monitor and assess the health status of individuals, ensuring timely communication with care providers.
  • Conduct face-to-face or virtual training sessions for family members and caregivers.
  • Respond promptly to emergency medical situations.
  • Oversee medication management, including reviews and assistance with obtaining prescriptions.
  • Maintain accurate health records and documentation as per company and regulatory standards.
  • Provide crisis assessment and support to individuals and teams as necessary.
  • Collaborate with program leadership and team members on individual support initiatives.
  • Ensure compliance with organizational standards for health, safety, and confidentiality.
  • Assist individuals in accessing necessary health services for various medical needs.
  • Participate in and attend relevant appointments and meetings as required.
  • Develop and update individualized health risk plans to facilitate appropriate medical care.
  • Support transitions of care between different healthcare settings.
  • Provide oversight for physician orders and appointments.

Qualifications:

Minimum Requirements:

  • Current California Registered Nurse licensure.
  • Professional license in good standing.
  • Valid BLS CPR and Basic First Aid certification.
  • Certification in crisis de-escalation and physical management techniques.
  • Valid driver's license and auto insurance.
  • Proficient in computer usage.

Preferred Qualifications:

  • Two years of experience working with individuals with IDD and behavioral health conditions.
  • Bilingual proficiency in languages such as English/Spanish, English/Mandarin, English/Korean, or English/Tagalog.
  • Experience in community settings with individuals in crisis.
  • Familiarity with California DDS, MHSD, and Regional Centers service systems.
