Job Summary

The Family Support Navigator position at Charles River Center is a dynamic role that requires a strong understanding of the needs of families with individuals living with intellectual/developmental disabilities and/or autism. As a Family Support Navigator, you will work closely with families to develop and implement personalized support plans, providing guidance and resources to help them navigate the complexities of the healthcare system.

Key Responsibilities:

  • Develop and implement person-centered goals in collaboration with individuals, caregivers, and families.
  • Provide information and referral services, as well as service navigation, to support families in accessing necessary resources.
  • Facilitate social and recreational opportunities for individuals and families, promoting community engagement and socialization.
  • Administer flexible funding to support families in meeting their unique needs.
  • Collaborate with caregivers and individuals in their homes and communities, providing support and guidance as needed.
  • Work with families to develop and implement support plans in response to identified issues, including educational, social, vocational, behavioral, and clinical needs.
  • Assist families in navigating public assistance systems for benefits and other resources available to families.
  • Organize and lead social and recreational events and programs for individuals and families.
  • Provide training and education to families on benefits, transition planning, and other relevant topics.
  • Draft and implement Family Support Plans and Expenditure Plans based on program policies and procedures.

Requirements:

  • Bachelor's Degree in a related field, or five years of experience working in the Human Services field.
  • Strong understanding of the needs of families with individuals living with intellectual/developmental disabilities and/or autism.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and customer service.
